Services

In addition to all of the many services we provide to patients of all ages at PHC, we provide several services designed specifically for older adults.

Medicare Wellness Visit

  • This annual visit is an important benefit of Medicare Part B that focuses on prevention. It is not a physical exam that you typically receive from your provider.

Assistance with Medicare

End-of-Life Planning

  • Do you know who will make health care decisions for you if you are unable to make them for yourself? We recommend that all patients talk to their loved ones about what matters most to you if you get seriously ill.  This is called advance care planning and it ensures that your wishes are documented in an Advance Directive like MyChoices or Five Wishes (available at your appointment).

Need help creating an Advance Directive?  PHC Social Work can help you every step of the way.  Call (406) 258-4789 and ask to speak with our social work team.

Frequently Asked Questions

What is an age-friendly health system?

A health system is age-friendly when it is expertly designed to coordinate all of our care as we age, while also making sure that our personal needs, values and preferences are at the heart of that care. Age-friendly health systems pay particular attention to;

  • Providing older adults with the best possible care
  • Reducing some of the specific harms that older adults face more often than others
  • Ensuring older adults, their families, and their caregivers are satisfied with the care we provide
  • Improving the value of care for everyone involved, including the professionals who make the care possible.
